Advance Care Planning

An Advance Care Directive states your wishes for end-of-life care and includes a durable power of attorney for healthcare and a Living Will. Planning for the future with an Advance Care Directive can give you and your loved ones peace of mind. If you want to learn more about issuing an Advance Care Directive, please contact Mary Ann Antonelli in our Quality & Resource Management department.

If you are concerned about an existing or potentially complicated ethical issue, you may request a consultation by calling the vice president of Mission and Ethics at ext. 63301.

For more information on recent changes in legislation regarding Advance Directives as well as educational materials and sample forms, please visit Foundations for Healthy Communities at www.healthynh.com.
