Caregiver Clinic
At the Roger Dionne, MD, Senior Center, we understand the importance of helping loved ones remain independent for as long as possible. The caregiver plays a key role in helping older adults remain at home and close to their families.
Providing around-the-clock care for loved ones who require assistance with daily living activities can leave caregivers and family members feeling stressed and isolated. The Caregiver Clinic can provide guidance and support to help you meet the needs and wishes of your loved ones.
The Caregiver Clinic is led by a licensed clinical mental health counselor who is trained to assist those who take care of older adults at home. The counselor meets with caregivers one-on-one and conducts a care-planning assessment to:
- Identify problems.
- Determine eligibility for assistance and services.
- Assist with the arrangement of services.
- Provide crisis intervention.
- Make referrals to medical professionals for additional services.
Once a care plan has been put together, the counselor will create a customized Caregiver Kit containing both informational materials and guides to resources and support services applicable to individual needs.
