The Greater Nashua Healthy Community Collaborative
The Greater Nashua Healthy Community Collaborative, formed in 1997, is made up of health care organizations, human service providers and public agencies that come together on a regular basis to address health issues that affect our community. They focus on needs that have been identified by the 2002 Community Assessment of Greater Nashua. One of these issues is prescription assistance, which was established to help their clients obtain prescription drugs.
“Through the work of the Greater Nashua Healthy Community Collaborative there has been an increased effort to educate our community about the importance of preventive medicine and screenings,” said Claudie Mahar, RN, MSN, Vice President of Hospital Services for St. Joseph Hospital. “Cardiovascular and PVD screenings are beneficial to our community because conditions such as diabetes and high cholesterol can be identified and treated before they result in a heart attack, stroke or other traumatic event.”

Milette Manor Parish Nurse Center for Wellness
The Parish Nurse Center for Wellness at Milette Manor is a collaborative effort between Neighborhood Housing Services of Greater Nashua, St. Louis DeGonzague Parish and the St. Joseph Hospital Parish Nurse Program. Milette Manor is located at 72 Vine Street and provides housing for (number) low - income, elderly persons. The residents have access to the wellness center's equipment, programs and screenings. St. Joseph Hospital's Wellness Center at Milette Manor, which is staffed by two hospital employees, also acts as a resource for area's residents and community members. This xx provides an opportunity for grassroots outreach programs to work together and help educate our community about wellness and health issues. For example, Heart Healthy Latino Cooking Night is a collaborative program offered by the Foundation for Healthy Communities, Nashua Health Department, NH Office of Minority Health, St. Joseph Hospital Parish Nurse Program, NH Minority Health Coalition, Nashua Soup Kitchen, UNH Extension Services, Nashua Boys Club and Operation Frontline/NH Food Bank. The program's purpose is to educate the city's Hispanic population about the increased risk of cardiovascular disease.
Screenings
PVD Screenings
This free screening is offered through The Cardiovascular Center at St. Joseph
Hospital the 3rd Thursday of each month. To schedule a screening contact
MedCall at 1-800-210-9000. Availability is limited.
Blood Pressure Screenings
This free program is offered at Milette Manor on Mondays, Wednesdays and
Fridays from 9 a.m. to 12 noon.
Breast and Cervical Screenings (Let No Woman Be Overlooked and Cervical
Cancer Program)
This is a free program, offered the 4th Wednesday of the month, provides free
breast and cervical cancer screenings to women age 18 to 64 who have no
health insurance of have insurance that does not pay for screening tests. There
are income guidelines to qualify for this service. For more information or to

Advanced Care Planning
Having an advanced care plan — which includes a durable power of attorney for
healthcare and a living will — gives you and your loved ones the peace of mind of
