Our goal is to treat our patients with respect and courtesy. We try to stay as close to scheduled appointment times as possible and ask for your understanding if your appointment is at all delayed. The practice of medicine is unpredictable, and sometimes our schedules are interrupted due to other patients’ medical needs.


  • We make every effort to accommodate urgent appointment requests as soon as possible.
  • Please call to let us know if you are delayed for your appointment.
  • If you are unable to keep your appointment, please notify us 24 hours in advance.

Office Visits

  • Please bring a list of all medications that you are currently taking.
  • Please check-in when you arrive. We ask that first-time patients arrive 15 minutes early to complete new patient paperwork.
  • We ask that you bring your current insurance information and card to assist us in proper billing.
  • Please request a referral from your primary care physician if your insurance plan requires a referral.
  • We suggest you wear comfortable clothing that can be removed easily if necessary.
  • Patients coming in for EMG examination of the:
  • arms (e.g. for ulnar neuropathy, carpal tunnel or cervical radiculopathy) should bring or wear a t-shirt or halter top.
  • legs (e.g. for sciatica, lumbar radiculopathy, low back pain or neuropathy) should bring or wear a pair of gym shorts. Women should not wear stockings. Please do not apply skin moisturizer on arms or legs prior to an EMG test.
